Please fill out this form to let us know if you are planning to attend the USS Augusta Decommissioning Ceremony. The form will serve as a security (pre-requisite) Access List for getting onto the base for the DECOM CEREMONY and possibly tours of "the 10 boat" on February 7th, 2008. Please don't forget your picture ID. We do not have the original social security list used for the Reunion in October and we do not have the original mailing list so please let us have a current email. You can send the #SS in two emails if you want.
